Adoption Application

We hate paperwork, too. But to help us keep track of all the people who want to give our dogs wonderful homes, we need you to fill out an adoption application. An application does not mean you are required to adopt – it just lets us know you are seriously considering a dog for adoption.

We recognize that you may find this application long but we want to ensure that your match is an ideal match.  Please note that we take Heartworm Prevention (HWP) seriously due to the high risk possibility in the Southeast.  If your current dogs are not using HWP, you application WILL be rejected.  We will verify with your vet that such prevention is being used.  Thank you for your understanding.

Please complete all applicable fields.
